How To Register As A Patient

If you are new to the area and would like to register with one of our GP’s, please check you are within our practice boundary and complete the online registration form via our website. If this is not available to you, please contact a PCA for a new patient questionnaire.

When you register with us you will be allocated a named GP. For continuity of care, we encourage you to see your named GP each time you come to the surgery. If you are not happy with your allocated GP you may request a change and we will endeavour to accommodate your request.
